Welcome to version 1.0 of the National Preference and Feedback Form. This form is required to be filled out by all athletes participating in the National Team Program.
Step 1: Personal Data
You will be required to fill out contact details for use of the management team to create online profiles for our new system. You will also be asked personal questions that will remain confidential between the GM (Rosie Everett - squad@dodgeballaustralia.org.au) and yourself (i.e. Sexual Orientation, Gender, Salary, etc) that will assist the GM in collating data about the team and making informed decisions for the group.
Step 2: Survey
You will be asked specific questions about the program(s) you are training in and about upcoming competitions available to WDBF NSOs. This will assist the management team to make informed decisions for the group.
Step 3: Feedback and Suggestions (Optional)
You will be able to submit feedback about the training(s) you have participated in. You will also be able to suggest any changes for the management team to review.
Step 4: Preferences
Here you will submit your preferences for National Team Selection. These preferences will be used at selections by the National Coaches and management team to coordinate the teams for the final playing squad for the competition.
