If you don't have a physical business address, list the location/venue that your organisation most frequently operates at.

Provide a short description of your organisation, who it caters to, what services you offer.

Please provide a 1:1 logo for your organisation (preferable as a .png).

You should be an owner or a representative authorised to apply for affiliation on the organisations behalf.

This should ideally be a business email.
